1. Place the provided foam in-between models.

2. Secure the articulator with a rubberband.

3. Wrap additional items in bubble wrap.

4. Place the articulator in the cardboard insert.

5. Place the articulator and insert in the box.

6. Place additional items in the box.

7. Fold the completed Rx and place in the box.

8. Close box and seal with packing tape.

9. Peel and stick preprinted label to box.

10. Arrange for Pick-up.
